Lets just buy the rights to CryEngine 3 and get a 300-man team to work full time on IS in a 40 story building.
I thought that if you shot someone through the brain it instantly shut off body functions so people would just crumple. The only reason I wouldn't like this in most valve games is because of how weird people with the human skeleton in HL2 looked on the ground. It just seemed unnatural.

You intergrate it into your own game engine you've made/bought. In the case of GTA IV, they used Rage. The Force Unleashed used a big mish-mash of Ronin, Havok, and Pixelux Entertainment's DMM. So technically it could be possible to have both Cryengine and Euphoria, though please correct me if I'm wrong.

To make ragdolls less silly, increase their weight. The limbs are too light, thus they flair around a lot.
It's probably possible to use Endorphin for your animations instead of Euphoria. I've seen people import the Valve skeleton in there and make them look like they got shot or some shit. No fucking clue how to do such things, though.

Oh oh, I've got another silly suggestion. When a player gets hit, why not blur his vision for a second or so. I'd imagine it'd be a bit of a shock especially if the bullet hits you in the vest and knocks your air out (for 7.62 at least). So he can't just run into a middle of a room to take bullets as he shoots the baddies. The guy who shoots first gets the advantage. And maybe have like a transparent ghost image too.

Take for example brothers in arms: [img]http://gscanner.info/wp-content/uploads/2010/12/6-350_2.jpg[/img] looks fairly realistic and non-intrusive, but still produces that "oh fuck i just got shot, better find cover" feeling.

Red shit on the screen is retarded. Why the hell would there be droplets of blood ON YOUR PUPIL and in focus? The smallest droplet of blood would be big enough to cover your entire pupil with red, and it wouldn't get in your eye anyways because you would blink it away. Maybe it'd make sense if you were wearing goggles or something, but then only if you got shot in the face. I think just some blurring and darkening/vignetting would be a lot more believable.
If you want to go for full realism, make the screen flash black every two seconds or so, for the character blinks. Also add a constant mini-game where the character has to keep balance. Realism does not equal fun.
I remember Rainbow Six: Lockdown where you would get bullet holes through your visor. Like when you were low on health. How the player could survive a round through the head remains a mystery.
