Adding a rad measuring item can possibly extend gameplay and improve the rad system. Having the whole rad ui initially gone, you will need to obtain the radiation counter or some sort to measure how much radiation you have taken and how strong the radiation is. Being able to know where the radiation is and how strong it is, would benefit the person that has obtained the item, rather then the person who doesn't and would have no idea where the radiation is at all, where as they could start taking radiation poisoning. This could alter the loot spawns from having everyone to just rush at the loot, and instead for the players to be careful and come heavily protected.
I could agree to this, on a couple of conditions:
1: There should be some way to know you are recieving SOME radiation, like the blurry screen seen in several (But for some reason not the current) versions of the game.
2: Being damaged from rad poison should have a unique effect, so one doesn't think they're being shot or bugging out or something.
3: It needs to be at least somewhat ergonomic. I don't want to have to carry it around with me, like the compass or watch in minecraft, when I'm in a dangerous radioactive zone.
(Though, I can rethink number 3, perhaps it should need to be carried, so it either poses a big risk to the player using it, or forces players to cooperate, one person holding the geigar counter/equivalent)
