What about making only one, but badass NPC as a start?
We need a hell of a name, not like "hunter", "predator" and such cliché ones. I am not partially good in it, so I just proposed one, inhabit-ant. Inhabit, cause it is so intelligent, like it knew the place he is moving around.
He would be a really well programmed predator, and one of its biggest weapons were traps. He would be able to play any sound (lore), making you run on occasions in the direction he wants to. Like there is a team of bandits, and plays gun sounds in the safe directions, so you may run into that bandit of teams. He would also play attracting sounds (not sure what are), or like someone is walking not far from you. He could light a lamp pretending to be a naked player, attracting you in dark, then get you from a safe place. He would cooperate with a mate, so he may distract you while the other guy stabs you from behind.
Being intelligent, he would use covers, run away on being damaged. He would not be the best killer, but can and will try to kill you one way or an other. Oh, he would be able to play recent VOIP chat sounds, scaring the shit out of you again. Making you scared, so you will hide in one little place where they form a team to take you down.
Here it is guys, an NPC proposal, good for getting into more PVP and PVE trouble.

Seems a bit too spooky and unexplained, but the idea of a revolutionary AI intended for ultimate spookings spooks my spooknerves.
Something clawing at my door would be bad enough.
That would be awsome but (I'm not a programer) , I imagine, something hard to code since map are procedural. Maybe on Hapis...
