I might be late on this, but the other day I realized occasionally if you open a gate, doesn't seem to really have anything to do with where you are standing, but there is probably about a 1/50 chance the wood gate will toss you rapidly and violently to the top of map and possibly disconnect you from the server before your body hits the ground.. then of course my body landed intertwined in all of my large spike walls, so retrieving my pack wasn't really worth it, however I did retrieve my back and none of my gear was there, just the starter rock, bandages, and torch. Anyways, wasn't sure if developers were aware, so just pointing it out, and maybe clarifying some confusion in the community as to how you just died, you probably upset the gate.

p.s.s. If you played Skyrim on the midnight release before any patches.. and you got to close to a giant.. you probably know the feels.
