How do I create a game that my friend can join?

I'm sorry if this a stupid question, but I'm having many troubles....I see no 'create server option' from my main menu. When I select a map for a new game and click 'start multiplayer' my friend is still unable to join. Yes, my ports are forwarded; ad yes, sv_lan is set to 0. I will be eternally grateful to somebody that can offer me some advice. Many thanks!
