Title shows it all. Heres a photo of my audio settings for windows 7. It was working one day next it stopped.

It only breaks on my headphones. It works fine on my internal speakers
Change from 4 speakers to headphones.

Still not working. Headphones work on other programs like chrome though. Is a restart of gmod required when changing. I doubt it.
Try restarting it. Also, is your volume in your mixer turned up for the sound device connected to your headphones?
Nvm! Fixed it. For some reason the volme thing on volume mixer showed it was enabled yet it was disabled.
