Invisible players (not playermodels), can see gun and scoreboard is messed up

There's a recent bug that I noticed on my servers where players would randomly go invisible, and show up as dead/spectating/spectator deathmatch on the scoreboard.

First person seems fine, ofc, but the player simply looks invisible.

Any suggestions? I verified my files already.

EDIT: solution I found is to reconnect the user.
