I have an issue whenever anybody dies from fall damage, world kill or something that is not a player, they can still talk to alive and would show as alive until they go to spectator.

It's probably wyozitageditor, as (I'm assuming) it messes with the chat.

I've been checking, the tag editor wasn't messing around with it however i have a chat rank thing i can't find anywhere.
Try it without the HUD switcher. Funny enough I realised I'm having the same issue and going off your list, that's the only addon we have in common. I'm currently at work so cannot doublecheck for you.
it's customcommands. somewhere in there you might find cc_chattags and i think thats what cause it

The old version or the new mysql one?
the mysql one got for some people this problematic.

The new one [u]needs[/u] MySQL so OP make sure you have that enabled.
