I created a dedicated server, and whenever I log on, this happens:

That's bad. It stays there the entire time, and I can't open anything, like the build menu (it pops up a new error that goes by pretty quickly).
What did I break/do wrong, and how can I fix it?

The problem was there the first time with no addons, but I've added Wiremod and PHX3.

Delete gamemodes/sandbox and base and update.
