Put it like this, it was a black and gray player model that look like Gordon Freeman, and had one arm up like he was holding a cup.

You can see it in third person when in cars as well.

It's player.mdl, and it's a placeholder for people with no playermodel when they connect to a server, if you could post the screenshot maybe we could help.
