I just need a short tut on how to install the mysql and hook it up.
You need to install tMySQL onto your server to allow it to interface with a MySQL server.
Next you have to upload the mysql tables to a decent MySQL host and put the login info for the database and user in sv_hooks where it says tmysql.initialize.
After that you have to import the tables that should have been given to you into the database (usually via PHPMyAdmin)
This is all assuming that your copy of PERP even works. You need the ASSMod addon to define users and to make the gamemode work properly. You will also BASSMod, pimpmyride, gm_slog and gmsv_gatekeeper.
What host should I use and by when you say tables you mean all the written codes inside the mysql?
