did you just use filter > noise?
the problem with that is that standard noise like this only differentiates by, like, one pixel. it makes noise itself look really shoddy -- especially when it's monochromatic like this
don't do monochromatic if you're applying noise directly to a picture, and try leaving at gaussain. there's other and better noise filters for especially photoshop (55mm's noise is AMAZING because you can set it to specific color and shading paths so it only affects certain color gradients which is very cool and nifty) but those cost cash which you don't want to do.
best way to do noise like this is to do different levels of intensity. make one layer with a little bit of noise and another layer with a lot and just erase the parts that seem too heavy.

also the lighting effect is very well done -- good job on that but once again the noise really cuts out quality because of this. the noise shouldn't even be affecting the coloring
