The joke is the heavy is astonished at the sight of a dispenser, the heavy seeing the dead medic makes him think that the dispenser has killed the medic.
So the heavy shouts "herp a derp monster." at the engie.

I love the text, it puts emphasis on just the right thing while not being too pretentious. The dead Medic really adds atmosphere and the emotion in the heavy's face is the best I've seen yet.
