Error when i kill someone

so for some reason when i kill someone it doesn't happen all the time but it has happened when i killed someone by landing on their head and when i got a head shot on them with a magnum i don't know what is going on

error is

Somehow I get the same error, what addons do you use?

It happens when you get killed by someone who has ' in their name.
