Technically, hexed means that it uses a different file name. Normally, it's done so you can use a custom skin and the default at the same time ("could you hex this Alyx skin for me?"). However, it's sometimes used to make one model replace another ("could someone hex this to replace Alyx?").

Generally, though, something labeled as "hexed" refers to the former, not the latter.
