I decompiled, changed Human Grunt model from Half-Life with Blender and ran into a problem:
After compiling, grunt's head looks like this:

This also happened to grunt's feet, but the problem suddenly disappeared.

Are you sure that each verticle has rigged to only one bone and bone weight is 1? If your verticles rigged to 2 or more bones (even with zero bone weights), they'll be rigged to origin bone when exporting to smd.

Yes, verticles of the head are rigged to the head bone and weight is 1.
