Wooden Head Syndrome, I believe they call it. I can't remember offhand how to fix it, but I remember it being a fairly simple fix. Either that, or you really have done something wrong. I'd search around for the fix and try that first.

This should fix the wooden face problem, this was valve fault on making a certain setting on the texture to do that for some reason.
