Twelfth from the bottom on the left. These are the achievements for Valve's new game Alien Swarm coming on Tuesday, and there is quite clearly an achievement with an image of the Heavy with an alien from the trailer on his head, with the achievement called "Hat Trick". A bit more than obvious that this is another event hat.
Also, the Cake up the top is worth noting.

I think it is just an alien that tries to hump your head. they put the heavy in there because TF2 has hats. A homage of sorts.
The alien likes heads and you shoot him off your team mate?
Anyone else think one of the aliens looks like a Gonarch from HL1?

You make it sound like VALVe created Max, and the only reason he exists is because he's he's a hat in Tf2.

Hah, I was waiting to see how long it would be until somebody said this. Honestly, can't you just have fun? They're free bonus hats and they let you play TF2 while showing your love for other games. Who really cares if they "shouldn't exist in the TF2 world"? They're fun and they continue to keep this nearly-three-year-old game alive.
I'm just glad that this hat simulator is war themed. Imaging a puzzle themed hat simulator.
