I think that has something to do with the fact that the class models have different face flexes and they result in slightly different facial expressions.

Disguised faces do not follow the faces of their disguised enemies, they are dependant on the spy disguised as them. So if the spy does a positive voice command, the disguise will smile and do the face a heavy would make. But at the same time, the person you're disguised as will make a negative voice command, resulting in that heavy frowning while yours is smiling

So you're saying that the disguised Heavy will have both facial flexes at the same time which will give us something like this?

My guess is that he was referring to how disguised enemies have incorrect idle faces. Most of the classes squint or frown a little, as they all have idle expressions when nothing is going on. However a disguised spy has the stock face pose, ala default gmod face. It's why more than often that teammate who shortly shanks you has an emotionless glare on his face.
