It downloads automatically.
...So how do you get to it? I've opened l4d2 and checked campaigns and add-ons and it isn't in either of them.
The DLC is downloaded and installed automatically, if you have an xbox you need to download it off the Marketplace. Its 560 MS points.
It should be under Dead Center in the campaign list.
