Did you buy it retail, or on Steam and then just add it to Steam?

Try this:
Go to the icon for Rockstar Games Social Club in the lower right hand toolbar. Right click it and idsable it starting as soon as Windows starts.
Then exit it and try running GTA 4 through Steam again.

dude GTA 43 uses real physics! its so awesome!
Add the "launchgtaIV" executable from
C:\Program Files\Steam\steamapps\common\grand theft auto iv\GTAIV
and launch the game from that.
