For me, it's defiantly the alien controller. They are pure creepers. You are walking down a Xen hall, you killed the grunts and got past the tentacles... what could go wrong now? *Bolted up the ass by silent Alien Controller*. Scares the hell out of me each time.
Nihilanth is the second-scariest to me (since it is a bigger version of the Alien Controller).
As for all of you, any game from the Half-Life series is allowed.

The Poison Zombie, without a doubt. Why? Because of that horrible gurgling sound and the fact they always hide around fucking CORNERS.

Ichthyosaur in HL1 by far, I had real trouble playing those parts of the game, it's linked to my aquaphobia I guess.

Ichthyosaur definitely.
I think it's the surprise mostly, swimming around minding your own business and suddenly you're missing a leg, and in HL1 you can't see them below the surface so if you're in the water you've got to use your pistol, crossbow, or grenades and satchels, which takes forever and is extremely dangerous. I hate sharks.
by hl2 though, zombies and headcrabs are all the creepiness it has left.

The Ichthyosaur, made me afraid to go near any large body of water in HL.
The Zombie in HL2, first time I saw one I freaked.

I hate Ichthyosaur, I noclip instinctively every time I see one of those in HL.
Gargantuas. I hate those motherfuckers. They scared the shit out of me the first time I played HL1 and I feel the same about them to this day.

Ichthyosaur made me build an instinct to NEVER step in water. I try to jump on to things to avoid being in the water for too long, or to bypass the water.

The female assassins in hl1 made me so paranoid for the rest of the game anytime it seemed too quiet.

I hated meeting a poison zombie because they were so disfigured and hard to look at. All those black things pulsating and changing him. I thought I heard one in the bridge part of Ep 2 and that was enough to freak me out.
