I can't believe I'm asking this, every time I have had this problem before it just kinda fixed itself without me doing anything.
So, I have tried everything, Patching, fiddling with the registry, installing to a non default location, using OBMM, disabling uac, running all oblivion related .exes as an administrator, still most mods don't work (and I say most because ONE mod has managed to work so far.)
If you could list anything I have missed above In this thread I would really appreciate it.

No, but I have never needed it before (I simply pasted over my old data folder from my backup drive)

Oblivion Script Extender needs to be used for the majority of mods, so that could be your problem, although you say it's worked before, which is odd.
Could you post the mods you use?
I'll give obse a try, maybe it'll fix it.

Although the main one I would like to get working now is 'Alternate start - revamped' which more or less allows you to choose where you start, not unlike an origin story in dragon age.

I think I may know that one.
You can choose things like wealthy in Chorrol, or in Skingrad, or a beggar's cave off of Cheydinhal, right? If I am, that sadly, didn't work for me either, so I dunno what to tell you.

So I'm assuming that other mods work for you fine?
You may want to reinstall each mod one by one, checking maticulously what each one requires and if it's incompatible with others. Also, get Oblivion Mod Manager and OBSE.
How are you starting up Oblivion? What .exe are you running?

Definitely not the majority.
Are you sure that the directory structure is OK, and the data files are checked?
