Check out Cryostasis, it's on $15 on steam and immensely worth it. A word of warning though, it's also collectively known as "Worst Optimization Ever : The Game"

System Shock 2 will change your perception on what is a good scary game and what is not.

Amnesia:The Dark Descend
Metro 2033 has a great atmosphere and moments that make you jump and make you tense. Doom 3 at times but its mainly a jump out of the closet type scare that gets old quick. The underground labs in the STALKER games, first time playthrough will create many brix.

Condemned criminal origins. I had to have my dad go up to a dead body for me at one part I was so scared. I was 15 then.

Cryostasis, was fun right up until you got guns, then it was too much like F.E.A.R. It also used the "monster-in-the-closet" scare tactic way too much. (Also, not to mention the hardcore rage-quit ending...) Even though you said no, I'd go with Penumbra, and when it comes out, Amnesia. Adrenaline rushes all around.

What the hell was up with the sniper rifle in Crysostasis anyways? It was there and it was useless because the zoom was like 20x or something and the enemies usually assaults you from about 2 metres at most.

Fallout 3 can be scary: I was walking around in a dark cave, I turn on my flashlight, and BOOM! Three Mirelurks, staring at me. Many brix were shat that night.
Penumbra Trilogy.
I like the unsettling feel in pathologic. But that game has more of a constant sense of dread and uncertainty than jumpscare moments.
I shat myself a couple of times in Bioshock, but all in all it isn't a scary game. [editline]09:05PM[/editline] Oh yeah the flash game "The House 2" terrified me, even thought it's only boo scares.
Penumbra has already been said. Therefore it's downhill from here. "The atmosphere! It is so incredibly thick!" I just love how it messes with your head, and how a lot of the scariness comes from your own imagination, as you unintentionally piece together scenarios.

Bioshock was a little scary at times, because of the atmosphere made it so fucking creepy. I can't remember the name of some other game that used to scare me.
I played the Doom 3 demo, and was a little scared in places. Probably because I read it was quite scary. Then I played the whole game, and didn't jump once.
