How Do We Know Chell's Name?

Just a quick Portal question. Where is the character in Portal's name revealed? How do we know that it's "Chell"? I just replayed the game and it's never mentioned by GLaDOS. Is it written somewhere? Is it from the ARG? Thanks for any input.

I think it's from her model's filename. Maybe the credits.
