It's very odd. Standing still, looking around, is completely fluent, however when I start running or simply walking the game freezes for a second, and then runs perfect, then freezes again after a few seconds and it keeps going this way.
Any idea why this is happening?

Specs? From initial thoughts I'd say that the euphoria engine is raping your processor.
This game is a bit unoptimized, I ould recommend posting your computers specifications.:dance:

It sounds like your processor is not enough juice for the unoptimized half assed port that is GTA4, or maybe you don't have enough RAM, or maybe your HD is just full and fragmented and it can't access all the files fast enough due to them being saved all over the place

Most of the files for just walking around would be already loaded, Remember this wasn't lag when accessing whole area's this is just walking around.
That i5, I might have found your problem. It's only a dual core for one, Game doesn't run so well on anything but a quad and well, It's a laptop. Laptops generally have extremely cut down parts compared to desktops, That wouldn't help much.
I'm getting lag aswell, heavy lag. Except I was able to play it properly before I reinstalled/formatted and it still happens on lowest quality possible?

Shouldn't the turbo and hyperthreading do anything about that?
And yeah, it's a laptop. What makes me confused is that I can run so many other games perfectly. I know GTA IV is unoptimized to hell (Dammit, XBOX).
Hyper threading, That won't really do anything at all. It's a useless gimmick to increase sales, and turbo boost, it generally isn't much of a boost so don't expect that to do much. You're playing on a laptop, Laptops aren't generally made for games.
It doesn't sound like low FPS at all, it sounds like stuttering. I can't remember exactly but I think I've heard/experienced this before. Try using CTRL+ALT+ESC (feels weird saying that instead of the norm) and putting GTAIV.exe Process to high priority, don't put it to realtime or the audio will crack.
If that doesn't work try: [url]http://support.amd.com/us/kbarticles/Pages/ATICatalyst912Hotfix.aspx[/url] apparently it's fixed it for a few people with 5 series ATI's.
Apart from that, look around on google and see if you can find anything, I'm almost 100% certain that I've seen this before, and it doesn't sound like normal lag at all.

Okay, I tried the priority-option, and I would be lying if I said it didn't kind of work. The lag is no longer when he starts walking, but rather EVERYWHERE. Not FPS drops, but now it can freeze pretty much anywhere in-game that involve player actions.
I set it back, restarted the game, and the same applies. I noticed though, that now the lag only happens when I press keys, so walking, sprinting, getting in cars, punching people. The lag only occurs when I do PLAYER CONTROLLED things.

Not sure but could it be your processor overheating? My i5 was overheating a few days ago on Starcraft 2 when I left the heating on and the windows closed. Some laptops can get pretty hot when running cpu-heavy games.

I've never gone higher than 80 degrees celsius on the processor, and I was doing heavy duty stuff at the time.

Try using the -memrestrict command line. I had a similar problem and it solved it.
How I use it, right click the GTA IV shortcut and go to properties, at the end of the target command line after the quotation marks add -memrestirct and either:

Gta iv is terribly ported. The best computers get lag.

Do you get the lag only when you press a button?
If thats the case, disable the "event Logging" windows service.

Yeah, any user input creates the lag. Just standing at one point idling doesn't.
Its not the specs, its something else, i played gta iv on a macbook pro with no problems and that has a core 2 duo and a 9400gt.
Well, I used to play using a Pentium D 3.2Ghz, which is no where near as powerful as a Core 2 Duo or i5.
As for was Fergeh said, typing Services.msc into run will open the Services manager, then right click on Event Logging and disable, if it is running.
GTA IV is too unoptimized and such.
I have a great comptuer which runs every game I have (including crysis) on max settings without lag.
Except GTA IV, that is
There is no such thing as Event Logging running in my Services.MSC
