One time in Battlefield: Bad Company, I was flying a helicopter over the enemy base, the only problem was I had almost no idea how to fly one, ending in my helicopter speeding towards the ground. I attempted to pull up and land and I did pretty well considering my helicopter was going down very fast.
But then my heli exploded, as confused as I was, the death indicator told me what happend, my helicopter had hit a landmine...
Missed knifing a guy on call of duty 4. For 2 minutes straight. :argh:
In project reality i was once APC driver when the gunner started to shout on his MIC; Stop! Terrorist outpost spotted. I had no idea and kept driving around. He raged out and kept shouting idiot. I said i didn't had an idea where it was and kept searching a bit further. I searched to far away from the squad we were supporting and they were all mowed down. Then i finally saw the outpost and destroyed it.

Killzone 2...just a bunch of fucking stupid deaths...IE, not shooting someone when I see them for no particular reason.
TF2, tried to land on a heavies head as a scout, and missed and fell off of a cliff and died.
When I forgot what team I was on in Call of Duty, and when a bunch've guys headed into the castle in that weird European map, they were about to hop into a car and saw me and killed me.
Not what happened to me, but i was playing cod4 and some cool kid thought it would be smart to t-bag me form behind. I turn around and just knife the kid and t-bag him back
I was playing Planetside flying a Gal the first time (A large transport ship) with a full load of people. I had been in one before and had done a drop, so I waited for everyone to jump down into the enemy base, and then hopped out myself. We all landed and were gathering ourselves when a huge explosion killed nearly everyone. The Gal fell straight down and exploded, it's supposed to be moving when the pilot jumps so it falls away from everyone.
Crysis just now, did a cheat that causes speed suit to got alot faster, managed to crash into trees all the time. Been crashing into trees for an hour or two.

One time I was playing CS 1.6 and I was on the catwalk above the garbage door in.. damn, forget the name. It's that map where the CTs start outside and the Ts are in a warehouse. Default map. Anyway, I jumped down to stab a T for the first time and I landed on him instead of behind him, I started stabbing downward at him but I couldn't get him, then he looked up and shot me.
CS ASSAULT, that's it.
Every single time I fail as Boomer, I feel embarrassed like hell.

Haha reminds me of almost the same thing in PR. Me and my squad was going to raid some outpost in one of those small choppers. So I got the pilot kit, everyone boarded, very cool and stuff. Then I proceeded to start the copper, which tilted back and exploded. The whole squad raged and kicked me from it :(
God, I swear, you always feel like a dumbass when you die in Battlefield: Bad Company.
I was hiding in a house, no windows nearby whatsoever, ready to kill any incoming snipers.
All it took was one 12 year old who thought driving tanks through buildings was awesome to kill me. While it is indeed awesome, he was on my own fucking team.
in cod 4, search and destroy, i just killed the last player in the other team(i was the last one in my team too), and i had to disarm the bomb, but i jumped down from a roof and got killed so my team lost the round and the whole match
Long long time ago, when I was playing Runescape, my level 48 character (I didn't really work on it, some guy gave it away when it was level 39) died to a level 6 mage.
TF2, double jumping to the log in one of the arena maps to get health, to slip off and fall to my death. Not once but twice in a row. :blush:

One of the more embarrassing deaths I've had was during an arena server on TF2, when I was playing as sniper I was the last person standing, but because I was cornered and didn't know I was I jumped off the small ledge to evade the pyros and died of falling.

BF2, one time me and this dude were sniping people up on a hill on wake island. I was pickin off people like crazy but I kept seeing my dudes getting picked off by another sniper. I spent like twenty minutes up there killing people and trying to figure out where the other sniper was. Eventually I ran out of ammo and got up to go. Only then did I notice the dude I had been sniping with the whole time was the other sniper, neither of us realized we were enemies. I started towards him to knife him.
He had laid a claymore.
I died. That's the most embarrassing moment of my entire video game history.
One of the most embarrassing deaths I had was when I was playing Neotokyo and I threw a frag which bounced off a corner, and airbursted behind my head- killing me and a friendly recon hiding nearby.
I felt cocky with a helicopter packed with people and I was like "watch this" and tried to do a flip, and hit up instead of down and nosedived into the ground, everyone jumped out and survived except me.
I was a sniper with my Razorback on TF2 and a spy was trying to knife my face in. So I turn around, hoping he'd hit my razorback and die*. (*be stunned so I could turn around and kukri the shit out of him.)
He missed his backstab and my razorback. The standard shank killed me.
The classical grenade tossing misstake.
