For a western airline, or one operating out of an Asian country like Japan, it'd have to be due to a failure of the tank to seal properly. 99.8% certain the FAA bans mid-air releases of those tank contents, for obvious reasons, and that's something I figure would be commonplace amongst the first world. But some airlines operating out of countries that aren't as strict about it still allow them to just vent the contents at will.

Always seems to be a story about some tiny village in somewhere land finding a piece of trash from civilization and wildly overestimating it. Last one I remember was that one where some Chinese village found what was, essentially, a Fleshlight and thought it was a mushroom that granted immortality.
