First thing first.
GTA IV is HORRIBLY optimised.
The problem i have right now is that barely anything is pre-rendered, like when i drive past a building, it's loaded then i drive past another and the building and the road etc. is completely invisible.

Change the draw distance in the options?
It's at full.
