Really? I could have sworn that the hitboxes lagging behind player models had always been a thing? I recall learning about this back in 2011.

Not the same degree, but they still haven't fixed maps having non-rotating background skies. They broke them in one of the earliest patches, yet they work in Gmod.

The hitbox lagging way behind players videos were iirc misinformed. In every game your hitbox lags behind you because that's just the nature of networking, that is why we have lag compensation to account for it. Lag compensation handles that, but this issue comes into play when your hitbox is actually misaligned or not properly following the same animations as your model so even with lag comp you aren't actually hitting what you think you are hitting.
I'm curious if this actually directly related to the notorious issue of headshotting people and seeing a blood splatter but it not registering or if its just something else that caused other weird situations here and there. I remember heavies being the worst for it...Headshotting a heavy that is walking slowly straight towards you while shooting and it not registering, when there is no real reason that it shouldn't register in good network conditions.
Seems weird that in all of their development time on the game nobody would have even coincidentally noticed this considering its pretty obvious in the video. I guess I like many others just chalked it up to a one off glitch every so often where the server was obviously seeing it differently to you.

As you can see in the video, sometimes the hitbox rotation doesn't match the player model's, hence why I assume it might have led to some inconsistency
Could MvM bots suffer from this misalignment as well?

Watching the attached video, the offset is egregious. I think I'll have another try at sniper in MvM now that this is supposedly fixed.
Even Bethesda isn't capable of having a major issue unfixed for 10 years, I'm impressed.

Sometimes, it makes me wonder if they ever bothered to fully playtest their things. With the Fallout games, I'm curious if ZeniMax is involved.

Their current game engine is the still the same ancient albeit upgraded one they used for Morrowind, yes, but what major bugs have they been unable to fix for 15 years? I'm curious.
I only know about the bug which duplicates the ripples effect from players moving in water, but that's very minor compared to completely broken hitboxes after changing classes.
That bug was a thing in CS: Source as well.
