At first I thought the parents were just idiots for letting their kids do this.
I still think that, but I'll cut a lot of slack because I've seen some of these games that are made purely for exploiting young kids, where each and every button will immediately pop a window up asking for confirmation to buy a really stupid item for like... 70$ in one go.

That is probably because the app store tells you if an app has in app purchases. And purchases can easily be turned off in the settings, so the parents are entirely to blame here for their kids racking up bills.
It should ask for the iTunes password every time you buy anything from it. That way it could be more safe but what do I know, I use android and it probably has the same problem too.

AFAIK, I think it already does that. At least for me.
And the issue is in-app purchases, you aren't directly buying from apple when you do that.

IIRC, they didn't add in-app restrictions until this shit already hit the fan. You could set restrictions on buying apps themselves, but IAPs were still open.
