I would say Fallout 4, but New Vegas already feels like Fallout 4 to me.

I read "Bethesda cease" and was expecting cease and desist on the Elder Kings mod for CK2. Not bothered about this though I think Skyrim's more than had it's time.
Bear in mind that 2011 was only when it was released. They've been working on it since Oblivion came out, and they probably have been working on this "new thing" since Skyrim came out.

Skyrims engine is based on Gamebryo. It's pretty obvious when you look at it from a development perspective.
I'll be the optimist here and say that with Three Dog's voice actor giving a hint of his return to the role, and with E3 2013 in a few months, it's without a doubt that it'll be a new Fallout game.
and hopefully it'll be a good one. Because they're not using Bethbryo. Either it'll be an improved creation engine, or something different.

Skyrim runs on Creation Engine, which is just a modified/renamed Gamebryo engine.
Bethesda did not make an engine, they tweaked an existing one.
They should do a game based on the other continents on Nirn, like Akavir.
My guess is they're either planning Elder Scrolls VI or Fallout 4. Unless they're planning a new IP, that is.

Skyrim's engine is still loosely based on Gamebryo.
Though it wasn't Gamebryo's fault that f3, nv and oblivion were ugly and had lots of glitches.

I had assumed it was like Quake to Goldsource level of changes.
