You have to take the knife and disembowel Henry's mom to get the key she swallowed, remember? Then use the key to open of the front entrance of the orphanage.

No no, you kill his old mother in the prologue with the cheese wire - hence "Horrible Henry". After the courtroom scene and the asylum bit you get assigned a foster mother. She's the one who works in the orphanage in her part time and you gotta get the key off her to get to the orphans. Naturally the bitch swallows it in an attempt to stop you so you have to do the gutting QTE which is kinda lame.
Thats odd. There's no video with heavy dubstep and filters of this game on youtube yet.
