I've heard the guy is an angsty, easily agitated unconfirmed neo-Nazi with potential mental disorders who at one point at a "realistic gore" option having used photos of actual bloody muck to include in the giblets.
Doesn't keep the mod from being good at all. It's free, it's not like you're giving him money in the process of playing it, and there are many people who fork their own versions of Brutal Doom to fix his shortcomings.

Oh well yeah. They're taking their experiences from the new Wolfenstein game and likely blending it into even faster gameplay. This is reflective of Brutal Doom's resurgence of attention on the 21+ year old game. It's also likely the best way for the game to stand out and how the fans will want to play it. They've tried, in Doom 3, for a more slow-and-steady horror and story approach, which was alright in it's own category, but not quite the Doom we knew and loved - which was action first and spooks second. However, going back to Doom 1 and 2, they were meant to be played as they were. Not updated to modern standards with over-the-top gore with all the inspiration coming out of the cheesy Doom comic. If you want to get real technical, Doom was actually suppose to be more like a sci-fi Hexen, with classes, inventory, bigger focus on plots, etc - before they boiled it down to what it became.

It's far from common practice to source your gore sprites from actual pictures of actual mutilated corpses. I don't know why in the world you'd ever think that was a normal thing for any community to do.

If I remember correctly, one of the Left 4 Dead 2 Comments Nodes mentioned that they studied photos of various diseases, but went with potato skin for infected.
Honestly, so far, this is still just another bland and disappointing release. The average person probably wouldn't even realize grenades were added unless they pay attention to their weapons and key bindings, and there's a multitude of problems and oddities. Like Classic mode needing to convert the Modern weapons to classic, thus fucking up your weapon selection mid-fight. I'm sure the Sperglord and Johnny versions are already in the works to convert and fix numerous things.
It's probably sad that the most enjoyment I've gotten out of this version is dicking around in Realism difficulty just because of how balls hard it is.
I'm looking into the real gore controversy, and there's so many people saying it's common practice to use [B][I][U]REAL[/U][/I][/B] gore in their video games. Citations? 0.

Headshots? Reloading? Recoil? I'm not sure what this is but it's definitely not Doom

People have their moments. It looks like a lot of the stuff he's done has been misunderstood for genuine racism or just in bad taste, but nothing actually harmful.
As for the game, I just tried playing with the Modern Realism settings and boy does it drag out Doom too long. I'll have to go back and try again with a Classic configuration.

I can handle dragged out, it seems. But with Classic, a single rocket blows away a Revenant or many other enemy types. A volley of Chaingun fire can kill most enemies but the toughest (aka Barons) in a couple seconds of sustained fire, and rip apart the mooks with ease. Even a single SSG blast can kill a Mancubus. The big problem is that Mark jacked up the enemy aggression so it's an extreme pain in the ass to even get attacks off sometimes without getting ripped up yourself, so corner-ducking is an absolute necessity to survive even 2 minutes.

Ironically I feel like original Doom should've either gone that route, or the Brutal Doom route.

I don't even understand what you mean by "identity problem". It's a arcade-style shooter from 93 treading in technology still in total infancy. It's more on terms with the japanese way of developing a action game in my opinion, gameplay and fun first.
They realized by cluttering the game with too much shit they would probably lose focus of the gameplay oriented design. Hell, imagine if they really did go the whole Tom Hall route and added all the crap in, you think John Romero would have had the genius idea to make the map design abstract in nature? Or would they have focused on making the maps more concentrated around the more advanced game elements.
In fact more shooters today need to simplify their thinking and mechanics and focus just on the gameplay engine and elements. Old fps games were great because all the maps were designed to go hand in hand with the simple gameplay engine, and you could create some really cool stuff just from experimenting around the way the game is balanced.
Most FPS games rarely have anything going for it to justify creative map making around the gameplay itself. Now it's just about the "adventure" and "immersion".

I find that last part hilarious because there's no longer any real sense of discovery in modern gaming in general (Achivement Unlocked: first mission! Achievement Unlocked: Found a secret! etc) and immersion is definitely in the shitter (press X to pay respects)
of course there's exceptions but most of the AAA titles feel really painful in this regard
