When it comes down to it, I don't think people like him feel they have a choice. I imagine in his mind it went something like this. "The shit's hit the fan. I'm scared, but I can still move. My buddy there is hit and if someone doesn't do something, he's dead. I can move, so I can do something. If I don't do something, I won't be able to live with the guilt." After that, bam. Mind's blank, and the body's practically moving itself. But I guess that's what separates people like him from most others, the ability to make a decision, stop thinking (Because thought is often crippling when you're afraid) and just act.

Most of the insurgents fighting in Iraq and Afghanistan are veterans of other conflicts: very well-trained, diabolically clever, and very, very technically proficient. Their failing is that some of the leadership isn't as good as it can be. You're thinking of the few borderline-insane fanatics who throw themselves into harm's way as martyrs. Those aren't even trying to do any real damage half the time, it's an entirely symbolic death.

They may not be as well armed or trained, but they fight until the last one is dead, and that can lead to entire outposts getting overrun simply because the troops at the outpost ran out of ammo.

The senior leadership may be Mujahideen die-hards left over from the Soviet invasion, but most of the rank-and-file are poorly trained wannabes with more guts than brains. You can find plenty of videos on Liveleak of roadside bombs failing to go off, basic lack of safety causing friendly fire casualties, and poor eyesight and discipline causing surprise attacks to fail entirely. The strength of organizations like the Taliban and Al-Qaeda isn't their individual training, it's their fanaticism and sheer resilience as an organization through careful vetting of new recruits and strong use of counter-intelligence techniques to resist infiltration.
