Sounds likely, while NK claim that they offer universal healthcare to everyone, defectors/doctors working abroad have said that it's only available to those who can actually afford it.

What do you say to accusations from defectors/doctors on exchange who claim that the DPRK does not offer universal healthcare, but instead only offers it to those that can afford it? In addition to that, what sort of reasoning do you provide for the hospitals being completely empty? (see BBC documentary for examples of what I mean).
