I'll start: When someone walks in on me using the bathroom. I fucking freak out. I know its my fault for not locking the door but in the moment I'm just like "WHAT KIND OF SORCERY IS THIS?" Worst. Feeling. Ever.
bees and spiders though I'm allergic to the former more than the latter.
Used to be afraid of washing machines and mower tractors. Was a weird kid. Nowadays I am afraid of wasps, bees and somehow afraid of some mushrooms. Don't ask me why.
Fireworks.
Dark giant sea monsters in the deep. Not like I believe in them, but the [URL="http://i.imgur.com/ZGMYcV0.jpg"]unknown[/URL] is frightening.
recently got my permit to drive, people who stop their car 3 inches behind mine and give me no space to put the clutch in without rolling back when i'm stopped on a hill. i always feel like i'm going to be referred to as 'that idiot who put his car in reverse and backed up into me'

Being trapped in tight areas, having no light to see anything and being lost, or being unable/difficult to breathe. For bonus points try combining any of those three.
Failing those I love. Also weird noises in the dark. Fuck that.
I developed some weird thought that there are demons everywhere I go as a kid. I get scared at any little sound or any little sudden movement. Hell, a large majority of the nightmares I ever had are related to them. This annoys me so much, especially because I know [B]they don't fucking exist.[/B]

Dying in my sleep. I don't know why, but it fucking scares me that I will just die without realizing it. But death itself also spooks me.
The thought that my life, after all of this struggle, end up being all for naught the day I realize I'm unable to reach my hopes and dreams. That I will go down with a whimper, without anything people can remember me by. My greatest fear is myself multiplied by time; I'm not good enough to do the things I wish I could.

The thought of anyone dying because of something I did. It's always there, at the back of my head. I can't say too much about my work, but it involves ambulances, army, police, fire department etc. and every time I'm doing something that's super-important that thought is there. It's not the following punishment that scares me, it's the fact that someone died because I failed. Though it's good too, because it causes me to always double-check then check again, and so far so good!
Wasps, hornets and aggressive ants. Fuck every one of those bastard fucks.
