They should make it so once you reach a certain level of madness through eating people, that you just lose control of your character and they become an NPC in the world not unlike the Yeti/Abominable Snowman from SkiFree

occasional moments where the character loses control at random and shoots someone near them, but it won't show the player :v:
I'd say that it shouldn't kill you on it's own, but drive you to the point where you want to suicide if there is no one to support you.
It'd be interesting if the game supported an insanity system, swapping player skins, halucinating that people are around, etc.

I'd love to see an insanity system where someone shoots a zombie but it turns out to be their friend.
Laughing should become more prevalent as the insane player gets closer to regular players.
Imagine how fucking creepy just looting a building middle of the night and hearing someone laughing outside would be
In space station 13 you can get "space madness", usually from the admins, but what it does is flip the view, make monsters appear that only hurt you and only you can see them, flash colors. I've never been insane before, but that's how I'd imagine it might be like.

make it so that if you eat enough of it you acquire a taste for it, meaning you can only eat human flesh until you die.

The reverse as well; zombies appearing to be injured or AFK players.
