Or anything. Since I've been busy these past couple of weeks.. I haven't had time to really play any games so I thought I'd play some Dark Souls 3 and possibly BF4 with my brother. Everything loads up perfectly fine but just crashes no matter what settings I choose. I've also tried windowed, borderless, and full screen. Some games are perfectly fine like Xenoverse 2 and possibly FO4 and Skyrim EE.
Tried to reinstall?
Multiple times. Even after doing a refresh... I'm getting the same thing

Certain games work perfectly fine like EVE, FO4, Xenoverse 2 and some others.
can we get your specs? and drivers version
375.70 Nvidia. 2600k, 20gb of ram, 760.
Downgrade the Nvidia driver to an older one. The most recent 2 drivers have massive issue's in BF4 and other games. Some people cannot even load windows.
iirc I was on 373.06 before
So kind of a update to this... I installed Overwatch since it's the free weekend and got a "your rendering device has been lost" error whenever I used borderless window and fullscreen mode
375.95 driver is out, maybe try that?
If it's not your drivers I dunno what you could do
Same. At this point I'm not really sure what it even is. I'll try the new drivers to see if it changes anything
tried the new drivers and still getting the same thing.
What actually happens when it crashes? Does the game just close and you are back at your desktop?
If it's a setting in the game for Dark Souls to to something like this:
"C:\Users\????\AppData\Roaming\DarkSoulsIII"
And open the GraphicsConfig.xml and change the Resolution and play with the settings. Turning some of them to "false" or "0" might make it boot.

The game plays and boots for 5 minutes then my entire computer freezes for a couple seconds. Game proceeds to crash while computer still freezes. I've managed to make Overwatch work by going into a menu when I get to to the main menu but borderless window doesn't work at all for DS3 and BF4.
I'm not 100% on this being a problem with it being either in fullscreen or borderless. It could be but I'm not convinced that it is.
Try to find the setting files outside of the game and adjust them, this is a good way to rule out if it's a problem with the game or your machine.
Like I said earlier.. These are the only 2 games (i guess OW kind of counts) where there are any problems. WoW, heavily modded FO4 (with some graphics mods), Dragon Ball Xenoverse 2, Tharsis, Skyrim Special Edition and Xcom 2 are perfectly fine. I can play each for hours and have no problems at all. I play all of them in borderless window. I'll try the settings file after I get back from work
Alright... I reformatted and Overwatch is still crashing (your rendering device has been lost). Haven't tried BF4 or DS3 yet.

If I turn the fan up to 48% it just makes a shit load of noise so I just leave it to auto.
