Basically I need to upgrade, there is fuck all room in my case, back in the day a 8800GT wouldn't fit in there, and the space has only gone down recently, I think.

Single slot cards have a bad habit of overheating... Without direct ventilation of a high-heat card, you are looking for a world of hurt. Just trust me. Don't.
On that note, define exactly what you mean by small... What are your case restrictions?

If your case supports dual slot cards then the GTX460 is pretty small.
