Well here's a thread where we talk about what scared us shitless when we were children, and also what scares us the most now. I'll start!
When I was a kid:
That Boss in Donkey Kong 64 that looked like a jack in the box, I would run off the edge and deliberately lose if it jumped towards me.

When I feel really alone in a game, with a sense of helplessness, along with being low on ammo or something. Happens to me all the time in S.T.A.L.K.E.R.

That boss is one of the reasons why I never beat Donky Kong 64 as a kid. I had to get my dad to beat him for me he scared the shit out of me.
Also, another rare made game that scared me as a child was Banjo Tooie, more specifically, the scene where King Jinjo got all zombified by Gruntilda. I was able to play that part but I had nightmares for like, a month. Thinking that'd happen to me.
Other things include:
007 Goldeneye (honestly no fucking idea, just remembered being terrified.)
Resident Evil Games
Legend of Zelda, Ocarina of Time (the graveyard temple place)
and any game actually meant to scrare you from that era.
Today, horror games still scare the shit out of me, I'm easy to scare, but I kinda enjoy it now. Except I still can't get through games like Amnesia. I end up quitting at every scare and not returning to it for months.

Just wanted to add on this, I love these kind of moments because while you feel helpless, you really start to rack your brain to figure a way out of the situation with what little you have. It's reasons why I like games like STALKER, Dead Space (the first two at least), Resident Evil and all that now.

Another Rare game, I think it's how Rare built atmosphere with music. Even if they didn't intend it to be scary it was still dark sounding and probably had a different effect on us as kids.

Perfect Dark was pretty scary back then. But I liked it's darkness back then. Maybe that's why they called it 'Perfect Dark'.

It took me too long to figure out how to beat it... Damn thing.
Oh yeah also I could never beat Majora's Mask because of the overall atmosphere and the creepyness and how dire the situation was.

I think I played it until I saw the moon and promptly went "nope". Actually didn't beat it until a few years ago when I bought a copy during a convention. It's still pretty damn creepy.
Oh, and Nightmare from the Metroid Fusion. Actually, Metroid Fusion is a pretty darn frightening game with all these tense situations and SA-X.
When I first got halo 3 I was too terrified to play any levels unless any AI partners where around because of the flood and shit.

There's also another game that scared the shit out of me recently. I got this game called White Day.(creepy korean game from like more than 10 years ago)
The atmosphere is done extremely well for a horror game, and the scares are random. Also more scares depending on your difficulty.

I played through that game so many times, but the encounters with SA-X and nightmare always gave me the chills.

Goddamn this game. It was one of the games me and my friend played while we were house sitting and he got this "great" idea to download a bunch of asian horror games to play during the night. This was easily the scariest out of the bunch.
Bowsers laugh in super mario 64 scared me when i was really young.

In fact, my friend who I mentioned before stopped playing SM64 for a few months as a kid because of this piano and now has a slight fear of pianos. This showed when we were playing all those games and one had a piano he refused to go near until I pretty much forced him too. He was then promptly eaten by it.

I never completed Ganons Castle Escape without the help of a friend when you see the Redead on that bridge. I just used to stand still and watch the Redead hoping that it wouldn't see me. However over the years I've learned to just run at them without thinking and take them down.

the flashback scenes in XIII unsettled me
for some reason the first Splinter Cell game used to scare me, especially the first level, probably because everything was so dark in-game or something, I could never play it by myself
i think i played the training mission a million times because it was the only thing i was comfortable with but fuck that lockpick training
oh and my uncle had a SNES with Super Mario Kart, the Ghost Valley tracks were probably my first gaming fears as a kid
Shadlebridge cradle in Thief. As a kid I was sort of scared in a thrilled sense for most of the stealth, but that was one level where I would just quit to desktop If I was spotted.
Also the lovely music with demented laughter, ghostly voices and crying children didn't help.

One of the first computer games I ever played was Doom, so I can safely say I wasn't really scared per se of anything in a video game.

theres one moment in particular that made me never continue playing half life 1. I believe it's the office chapter, and as you walk up to one of the doors a zombie busts its hand through. I never played half life 1 again after that until a few years later when I had become older
