So I built my pc 3 years ago.. and it's really starting to slack.
I'm just wondering what would be the most crucial to upgrade and what to upgrade it with.

Being that you can carry it over to your next build a new gpu such as a 7950 would be a good upgrade.
A new graphics card I'd say. You could also upgrade to a newer processor (i5 4670k/i7 4770k), but you'd need a new motherboard as well and it would probably cost more. Also you'd likely see a bigger performance increase from a new graphics card. Try a GTX 650 Ti Boost, a radeon 7870, a radeon 7950 or a gtx 760. It really depends on what you want to spend.

At the moment a 7950 is the best price to performance ratio.
A GPU upgrade will be a better choice, I've got a i7 965 and it will probably last me another year or two.

Also I think I might need to replace my harddrives and ram, both seem to be dieing...
Try and squeeze out performance in your CPU and overclock it, you can easily achieve 4GHz stable, maybe even 4.2-4.6GHz if you have the proper clocks. Ensure you have an aftermarket CPU cooler that will keep it cool, (I'm sure you already have one with that build).
Other than that, you have good memory times, maybe a bit low in frequency, however you can overclock that if you want, but to a very limited number. Purchasing new GPU(s) will definitely improve your gaming performance in your computer.
You don need to upgrade really, unless there is anything you cannot run.
The processor still holds up pretty well today, I would just get a new gpu. The 7950 is a great card especially for the price.

How big of a performance boost do you think it'd give me over my 5970?

I'm not to sure how well your 5970s preform but at 1080p a 7950 can run just about all modern games at max settings.

Thanks for that link, that does seem like quite the improvement.
Your HD5970 is still a great card. The only thing it falls short on is DX11 performance due to a lackluster tessellation engine.

The HD7950 is better in some benchmarks, but worse in others. In games that can utilize dual GPUs properly, the HD5970 will usually be better. In games that work better with a single GPU, the HD7950 will be better.
