Locked out of opinion survey thing while applying to job?

I wasn't quite sure how to describe this in the thread title or if it's exactly in the right spot. Basically, I'm applying to work at Urban Outfitters (yes, girlslaughing.jpg), and I applied to a bunch of stores before they sent me a link to fill out their opinion survey thing. This first happened in August a few weeks back and that was when I had a shitty internet connection. I tried to take it TWICE and both times it timed out on me. I'm guessing the second time is the last straw with it, because then I couldn't login after that. Since then I've applied to more stores, and they keep sending me the link to this survey thing. Their survey by the way, is very transparent, easy to get by, so the fact that they use it anyways is ridiculous. Google has not provided a SINGLE useful answer. In fact it hasn't given me any at all. I'm at the end of my rope for this.
I just wanna know if anyone has ever had experience with this sort of thing, and if there's anything I can do. I'm gonna go down to that store probably next week to see what I can do, since that's my best option, but goddamn it I am so fucking pissed. It might be embarrassing to ask them about it too. I already did a video interview for the store and everything, it's all lined up to be looking good. I really just want this fucking job.

Don't be embarrassed, it's a technical glitch. It's their responsibility that all candidates get an equal shot at applying for the job.
But I mean, I don't know. I guess while my connection was out it actually just timed out? I'm just really fucking scared that they'll tell me "welp. them's the brakes fag try again in 6 months we can't do anything about it you timed out lol" cause that would really suck. I'm desperate for this job specifically.
Try doing it at a library computer? Maybe they would have better connection.
Or call them about it?

Well, the problem isn't my connection now. It's the act that the website itself has locked me out (by my SSN of all things, I am fucking nuclear locked out)
Are you sure that is actually their site?
I haven't seen anything like this before and it sort of smells like phishing considering it asks for your SSN.
I also don't understand why a clothing shop would require you to use a SSN based quiz online from what appears to be a third party.

All job places require a SSN. They use it to verify work status and shit. Plus they run credit checks, background checks, etc.
I had to get fingerprinted for my job.

On top of that, its actually illegal for an employer not to keep that information on file. They need to verify that you are a citizen, on a work Visa, etc and that they make copies of documents like your SS card, birth certificate and such.

If I can't get any answers from Urban itself (that is, either of the two locations I'm going to call), then I guess i'll have to contact them. Thought I suspect this may be a rule that pertains specifically to the company itself, and not the people running the survey service.
If you Google "opse biz" - two words, otherwise you just get the site - you can find tons of craigslist employment ads with login information for opse.biz. I was curious what sort of opinion survey an employer gives to prospective hires, so I located a SSN that belonged to a deceased person, and logged into the opse.biz site as Leeroy Jenkins, trying to get hired at Piercing Pagoda.
68 questions to answer! But after about 10 or so, it was crystal clear that this was no opinion survey. This is a form subjects fill out to participate in a computerized psychological evaluation. OPSE will tell your employer whether or not you're a latent psychopath, or a troublemaker unafraid to talk back to The Man, based on your answers to this quiz.
I recommend only answering "slighty agree" or "slightly disagree" no matter how strongly you actually feel about the question. People with strong opinions are potential management headaches.

I've already taken the survey before but I just used that name since it's easiest to so it that way. My main problem is that even though I passed it around the beginning of the year, it's been more than 6 months ago.
An update on the situation: I went to 2 different locations, located relatively close to each other. The fist one being the location I'm not mainly focused on. I asked them if it would be possible to get an interview despite not being able to take the survey, and surprisingly, they answered YES. Then, I gave them my name and phone number. Then I went to the main location I'm trying to work at.
I've actually called and been here enough times to the point that I already have a sort of acquaintance relationship with the manager, who's a super cool dude. So he sees me and already knows me, naturally. We talk and I come to find out that he's actually sent multiple emails regarding the situation and included my information to see if the situation can be remedied. [I]No one knows what to do.[/I] He's told me that he's literally never heard of this type of thing happening, and that all the replies he's gotten have been "I don't know"s and overall question marks.
[I]What the fuck.[/I] Luckily they clearly see how passionate I am for this job so if the survey situation gets solved then it's looking more like a shoe-in. For now I'm gonna have to take matters into my own hands and call their tech support.
