Before you can call it an attack you have to have good information that it was an attack and not an accident. People have heart attacks or pass out without warning sometimes, it's just less reported.

In Germany it's standard to only report the events and not speculative reasons (so neither 'accident' nor 'attack') until the police says anything about it, though.
