The motherboard is a [b]P9X79 WS[/b] PSU is an [b]X-1250[/b] Flash drive was a [b]MKNUFDMH8GB[/b]
Could be a number of things. I had the same issue with an old computer of mine years ago. It rooted all the way back to a faulty installation of my OS. But let's not jump to any conclusions.. Have you tried plugging it in to other USB ports? Have you tried plugging it in to a different computer to see the problem persists?

It worked fine on my other PCs, but soon after trying to get it to work on the problem setup, it died, now it wont even work on anything ( it worked just fine for over a year ) [editline]2nd October 2012[/editline] it's made me hesitant to buy a new one for further investigation since it might just die again
Update the BIOS

It could cause problems registering the flash drives
reason i ask is because the drive seems to have died, doesn't work in any other PC's after trying to get it to work on the problem mobo
