I was trying to be funny.
Pull out the power supply after grounding yourself and open it up and check the fan-blade for proper clearance like anyone with a brain would first.

Turns out it's coming from one of my ATI cards.

If you have a warranty still, either RMA it or ask them to ship you out a new fan assembly. If it's not under warranty, take the shroud off and pull the squirrel blower off and put some lithium grease in the back of the motor where the shaft sticks out and spin it a few times so it gets everywhere.
If that doesn't work, there should be a circular clip on the shaft. Carefully remove the clip and pull the squirrel cage out and clean everything to remove dust and such and then put lithium grease on the fan shaft and put it back in and spin it a few times. Put the clip back on and you should be good to go.
