Smile dog is just wrong on a base level. Dogs don't look like that. They don't smile like that they aren't that colour they don't stare at you like that. The logical parts of our brain are overwhelmed by the older, more archaic parts that scream at us "This is wrong, this is unnatural, this is a violation of my perception of this world BE AFRAID OF IT."

I think people mostly consider it scary because of the creepypasta accompanying it. I have to admit, it was pretty well written and gave the dog thing quite a menacing character. It's probably the only creepypasta I liked. TLDR version of it, a reporter catches wind of a bunch of suicides or something and decides to investigate. He finds out from some woman that they all saw a picture of the dog, herself included. She explains to him that whoever sees a picture of it is repeatedly visited by the dog in vivid nightmares in which it instructs the person to show the picture to more victims in order for it to leave them alone. Most of the people go insane and kill themselves and it's implied that even after showing more people the picture, the dog still torments them.
Some of that might be off though because I haven't read it in a while but that was the jist of it as far as I remember.
