As long as it fits between two objects made of bread it's a single sandwich.
Unless they're split into different stacks it's just one sandwich. The Big Mac, despite having a third piece of bread, is still only one sandwich, which is why it's referred to as a singular object.
I feel like the key is to have something between any two slices of bread in a sandwich, lest it become two sandwiches stacked on top of each of each other.

The patty isn't contained within the tight cradle of a proper bun, it's not the cohesive single food item that we all associate with the sandwich. It's precariously balanced in between the grilled cheese sandwiches. Can we call such a thing a single sandwich? I mean where does it end? Would a pizza with marinara on both sides be considered two pizzas? Would two hotdogs pressed next to each other with a third sausage in between their buns constitute a single 'dog?
