it means "I am not guilty of what I have been charged with"
I think the general consensus is that you always plead not guilty, I heard that pleading guilty is not nearly as simple as pleading not guilty.
Didn't he not plead anything, so the Judge entered a plea of not guilty for him?

He isn't guilty because he's insane. If you're insane you can't be guilty because there is no way to ascertain whether or not your intent was to commit the crime.
I hope he gets sentenced to a mental health facility; where he belongs.

Your intent can be to commit the crime even when pleading insanity, but your faculty for understanding the repercussions of the crime may be lessened - by a whole damn lot.
Following your logic, I can kill a bunch of people and then say "but I'm crazy" and then act all crazy to "prove" it.

If your understanding of the repercussions aren't correct then your intent was, in your mind, for something else - is it not? Murder is taking someone's life but if you don't understand that you're ending the person's life you can't really be called a murderer. (Hypothetical)
If you were in medieval Europe and you stabbed your family to death you could plead not guilty on account of possession by witches, and the friendly neighborhood hag would die. If you are not in possession of all of your wits and do something caused by something that you can't control (i.e. mental disorder, not drugs or alcohol) you get a lesser charge.

I can't speak to his mental state but everything that is known about him points to schizophrenia, depending how bad his case was it's possible that he had no idea he was even firing a weapon. Or he could have thought that it was WW2 again and he was shooting Nazis. Or it was the future and he was shooting aliens. Or he was in 'nam. Who knows
