This first happened playing age of chivalry, and it's been getting worse overtime.
Random colored squares or lines happear on my screen, then the whole system blocks going in a black screen -> random lines loop, and forces me to force a reboot. Sometimes i get the "vpu recover has reset your graphics accelerator" error, but it happens rarely. First it started happening randomly while I was playing after some minutes, then it's gotten worse to the point where first i couldn't start any game without it happening, then even youtube videos or normal browsing were enough for my video card to fail. Then I tried to clean reinstall my video drivers, and I thought I had fixed it untill a day after the same thing is happening again

It's probably overheating. Try cleaning out your heatsink and see if that helps.
Yep. That's overheating. Open the computer and remove all dust.
If that doesn't work it may be the fan.
Run HWMonitor, keep it running while you play a game for 5-10 minutes, then take a screenshot of HWMonitor.
